How do i get a Medical Marijuana Card?

I need to know how i would be able to get a Medical Marijuana Card when i live in Illinois. I've heard that I can travel to a few different states that i dont need to live in, and then i can get a card there, but do i have to see a doctor in that state i travel too and then what? Fill out an application for the card in that state? I'm 18 and i also need to know if my disorders would be enough for me to get a prescription from a doctor. I have spherocytosis which also leads to me having Anemia. I have to take Folic Acid everyday for the rest of my life and two years ago i got my spleen removed because it was extremely enlarged since birth and ive been taking penicillin everyday twice a day and i have to do that for the rest of my life also. I also have ADHD and am currently taking 40 mg Vyvanse everyday. All of that stuff, but i also think i have some anxiety issues because there are some times that i feel extremely "trapped" alittle and i cant breathe. Especially when im in a confined area or in large groups of people. Im pretty claustrophobic also. I just need some advice from anybody. I also would not really want my parents knowing either so how do i go about doing that?
